To thank you for being a faithful patient we are offering the Whitening for Life program. To qualify for the Whitening for Life rewards program the following conditions must be met:
*You must be at least 18 years of age.
*You must consistently make and keep your scheduled 3, 4 or 6 month hygiene appointments.
*You will need to have custom made whitening trays. If you do not already have them they can be made and purchased through our office at a discounted price.
*You will begin receiving your whitening refills at your 3rd consecutive exam//hygiene appointment.
